An unannounced monitoring inspection was conducted today from 11:20am-2:10m. There were 28 children directly supervised by 6 staff and all classrooms were in ratio. The physical plant, programming, menus, 3 staff records, 6 children?s records, evacuation drills, injury reports, emergency supplies, and policies were inspected. The director advised no medication is on site today. Children were observed eating lunch which consisted of milk, whole wheat bread, turkey, hot dogs, mixed fruit, and corn and napping. Infant care including feeding and diapering was also observed. Positive interactions between children and staff were observed during this inspection. There was an abundant supply of age appropriate materials for the children. Areas of non-compliance are identified in the Violation Notice. Standard #: 22VAC40-185-140-A Description: Based on record review, 2 of 6 children's records did not contain documentation of a physical examination within 30 days of the child's first day of attendance.
Evidence:
1. Child #4's first day of attendance was 12/18/17 and did not have a physical examination on file.
2. Child #6's first day of attendance was 3/18/19 and did not have a physical examination on file.Plan of Correction: I already emailed the parents and requested copies of the physical examinations.
Standard #: 22VAC40-185-160-A Description: Based on review of 3 staff records, 1 staff record did not contain documentation of a tuberculosis screening or test submitted no later than 21 days after beginning employment.
Evidence:
1. Staff #1 (Date of Hire: 5/10/16) did not have a tuberculosis screening or test on file.Plan of Correction: I will have the staff get a copy of her TB screening and place it in her file.
Standard #: 22VAC40-185-240-D-5 Description: Based on record review and staff interview, there is not at least one staff member who has obtained instruction in performing the daily health observation of children within the last 3 years.
Evidence:
1. 3 staff records were reviewed and no staff had Daily Health Observation (DHO) training.
2. The director stated no staff have had DHO training within the last 3 years.Plan of Correction: I will have several staff do the online DHO training.
Standard #: 22VAC40-185-340-F Description: Based on observation, children under 10 years of age were not always within actual sight and sound supervision of staff.
Evidence:
1. Two children were observed leaving the Preschool classroom (Ages 4-5 years) unaccompanied by staff and entering the Infant/Toddler classroom and asking for toilet paper. The children were then observed walking back down the hall and returning to their classroom.Plan of Correction: We will make sure we have adults watching the children when they walk in the hallway.
Standard #: 22VAC40-185-560-F Description: Based on review of the posted menu and observation of the children eating lunch, the center did not indicate substituted food on today's menu.
Evidence:
1. The posted menu for today's lunch stated milk, whole wheat bread, turkey, strawberries, and green beans would be served. Observation was made in the Preschool and Three's classroom during lunchtime and the children were eating milk, whole wheat bread, turkey, hot dogs, corn, and mixed fruit.Plan of Correction: I have updated the menu to include today's substituted foods.
Standard #: 63.2(17)-1720.1-B-2 Description: Based on review of 3 staff records, 1 staff record did not contain the results of a fingerprint based criminal background check.
Evidence:
1. Staff #1 (Date of Hire: 5/10/16) did not have documentation of the results of a fingerprint based criminal background check on file.Plan of Correction: I will call on Monday to follow up on getting the results emailed to me.
