An unannounced monitoring inspection was conducted from 7:10 - 8:10 am with staff. There were seven children upon arrival, ranging in age from five years to ten years of age, present with two staff supervising. The children were observed playing in a variety of free play centers including dolls, blocks, cars, cards, and hula hoops. Four children's records were reviewed and in compliance. Two staff records were reviewed offsite at the Massad central office and were in compliance. The last emergency escape drill was conducted 9/27/19 and the last shelter-in-place drill was conducted 12/20/18. One emergency medication was reviewed and in compliance. Standard #: 22VAC40-185-340-F Description: Based on observation, and staff interview, not all children under 10 years of age were within actual sight and sound supervision. Evidence: Upon arrival, the LI observed six children in attendance, however staff stated that there were actually seven children present. Staff stated that child #3 (age 5) was using the restroom. The restroom is down the hall and not within hearing distance of the cafeteria. Plan of Correction: Staff immediately ensured that the next group of children using the bathroom were accompanied by staff, and will continue to do so.
