Comments:
A supplemental health and safety inspection was completed on October 18, 2019 from approximately 11:00 a.m. to 12:00 p.m. in response to specific compliance requirements the department was made aware of via a self reported serious injury report form. Staff and children were observed and pertinent children's records and injury reports were reviewed. The assistant director was available for the inspection and was present for the exit interview at which time the inspection findings were reviewed.

Violations were found during the supplemental subsidy health and safety inspection and are identified on the violation notice. The self report is supported for reports.

Standard #: 22VAC40-665-550-2
Description: Based on review of records, the licensee failed to inform the department's representative as soon as practicable, but not more than two business days, of any injury to a child while under the vendor's supervision when a referral is made for treatment from a medical professional.
Evidence: 1. A serious injury report form submitted to the department on September 13, 2019, indicated that Child #1 obtained an injury at the center on September 10, 2019, in which medical treatment was sought from a medial professional that day. Therefore, the department was not informed within two business days as required.
