A mandated Renewal inspection was conducted today. There were 23 children present with 5 staff. Children were observed painting their names with feathers, coloring and making Rainbow Fish, playing with trucks, washing hands, toileting, having lunch, setting up for nap, and sleeping. Reviewed 3 children's files, 4 staff files, Fire and Health inspections, emergency evacuation supplies and log, medications and authorizations, attendance, menu, and physical plant. Evidence:
1. Child#2 (start date 8/19/19) did not have documentation of the name, address, and phone number of two designated people to call in an emergency if a parent cannot be reached.
2. Child#3 (start date 10/24/19) did not have Center's blanket permission slips for trips to the playground, library, etc.

Evidence:
1. Staff#4 (start date 8/29/19) did not have documentation of the address for 1 emergency contact.Plan of Correction: Corrected at time of visit by Staff completing the form.
Standard #: 22VAC40-185-240-A Description: Based on record review, staff did not receive orientation training by the end of their first day of assuming job responsibilities.
Evidence:
1.Staff#1 (start date 8/12/19) did not have documentation of Playground Safety training.
2. Staff#4 (start date 8/29/19) did not have documentation of completing the Center's Orientation training.Plan of Correction: I will make sure forms are filled out completely
