Comments:
An announced monitoring inspection was completed on 10/28/19. There were 19 children present with three staff directly supervising and the director and additional staff person present. Five children's records and five staff records were reviewed. There were no medications at the center.

The inspector arrived at the center at 1:10 pm and departed at 4:00 pm.

The children were observed during outside time, during nap, during story time, during afternoon snack, and during departures.

Violations:
Standard #: 22VAC40-185-160-A
Description: Based on record review, the center failed to ensure that each staff member submitted documentation of a negative tuberculosis (TB) screening or test within 21 days of employment or volunteering.

Evidence:
1. The record for staff #4 was reviewed. Staff #4 had a date of volunteering documented as 8/5/18 and a hire date documented as 12/11/18. The staff person's TB test was completed 6/28/19. This was not within 21 days of employment or volunteering.

Plan of Correction: The director will ensure that all TB tests or screenings are completed within 21 days of employment or volunteering.

Standard #: 22VAC40-185-520-D
Description: Based on record review and interview, the center failed to ensure that there was written parent authorization noting any known adverse reactions for insect repellent that had been used.

Evidence:
1. There was a log observed noting application of insect repellent for several children. The children had written parent authorizations that did not note any possible adverse reactions.

Plan of Correction: The director will develop an addendum to send to parents of children using insect repellent to have them document any adverse reactions, if any, the child has had. The director will add the adverse reaction statement for parents to complete on the enrollment forms for the next school year.
