Comments:
An unannounced monitoring inspection was conducted from 7:30 a.m. - 8:15 a.m. There were 47 children in care being supervised by 3 staff members. The children were observed participating in center play and cooperative games. Breakfast served today is sausage, egg, waffle, apples, and milk. Six children's records were reviewed and in compliance. Staff training was reviewed for the three staff members on site. Seven medications were reviewed. The most recent emergency escape drill was conducted 10/01/2019 and the most recent shelter in place drill was conducted 09/18/2019. Standard #: 22VAC40-185-240-D-1
Description: Based on review of three staff training records and interview it was determined that there was not always a staff member on site who had Medication Administration Training (MAT) upon agreement to administer medication to children. Evidence: Seven emergency medications were observed at the facility however, there were no staff members on site who were MAT certified during the inspection.

Plan of Correction: Staff member will remain on site with the right certifications.

Standard #: 22VAC40-185-240-D-5
Description: Based on review of three staff training records and interview it was determined that there was not always at least one staff member on duty who has obtained daily health observation. Evidence: Training was reviewed for the three staff members on site who did not have documentation of completing daily health observation training.

Plan of Correction: A staff member has daily health and we made sure certifications are at the site.

Standard #: 22VAC40-185-510-A
Description: Based on review of medication it was determined that not all policies in regards to medication were followed. Evidence: Six of the seven emergency medications reviewed for Child A, B, C, and D did not have written parental authorization forms on file.

Plan of Correction: Train and send staff to MAT training. Until then make sure a staff is there who has it.

Standard #: 22VAC40-185-510-N
Description: Based on review of seven medications it was determined that not all medication was returned to parents upon expiration of authorization. Evidence: The authorization form for an emergency medication for Child A expired on 09/17/2019 and was still on site.

Plan of Correction: Make sure all staff know not to keep expired meds on site.

Standard #: 22VAC40-185-530-A
Description: Based on review of three staff training records and interview it was determined that there was not always at least one staff member on duty who has obtained first aid and CPR training. Evidence: Training was reviewed for the three staff members on site who did not have documentation of completing first aid and CPR training.

Plan of Correction: Make sure there is a staff member on duty at all times who is CPR/First Aid certified.
