An unannounced inspection was completed on October 23, 2019 from approximately 10:30 a.m. to 12:45 p.m. in response to a self-reported incident received on August 30, 2019 regarding forbidden actions. The investigation was initiated on August 30, 2019, collateral interviews were conducted off site on August 30, 2019, September 11, 2019, and September 26, 2019, documentation was reviewed on September 26, 2019 off site, and the investigation was completed on October 23, 2019. Staff, children and classrooms were observed, records pertinent to the report were reviewed, and staff were interviewed on site. The owner and director were available for the inspection and the director was present for the exit interview at which time the inspection findings were reviewed and an Acknowledgement of Inspection form was signed and left with the licensee. The self-reported incident is not supported.
