An unannounced monitoring inspection was done today 10/31/2019 beginning at 6:25 A.M. and ending at 8 A..M . There were 16 children present today. The children were observed today during arrival, table games,and free play. During the inspection five children's and four staff's records were reviewed.
Standard #: 22VAC40-185-60-A Description: Based on review of five children's records, the center failed to ensure that all records were complete as required by the standard.
1. Child # 1 was missing the address for one of the two emergency contacts.
Plan of Correction: The site director will obtain the missing address and monitor all records.
Standard #: 22VAC40-185-340-C Description: Based on observation, the center failed to ensure that during the stated hours of operation, there always shall be on the premises and on field trips when one or more children are present one staff member who meets the qualifications of a program leader or program director and an immediately available staff member, volunteer or other employee who is at least 16 years of age, with direct means for communication between the two of them. The volunteer or other employee shall have received instruction in how to contact appropriate authorities if there is an emergency.
1. At 6:28 A.M. the second staff was ill and left the program. This left staff # 2 alone with eight children until 7:10 A.M. There was a cook for the elementary school that arrived at 6:50 A.M. but was not informed of being the emergency person with direct means of communication between them.
Plan of Correction: In the future all staff will remain until the back up staff arrives. Site director will monitor all staff schedule.
