Comments:
An unannounced Subsidy Health and Safety Inspection (SHSI) was conducted today from 9:00am to 1:30pm in conjunction with a monitoring inspection (see that inspection report for details). The two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Five staff records were reviewed for documentation of the mandatory VA Preservice Training, First Aid and CPR training. There are children currently enrolled with diagnosed food allergies and their records were reviewed for allergy care plans. The last lockdown drill was documented on 12/5/18. Standard #: 22VAC40-665-520-B-11
Description: Based on review of documentation, the center did not obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction. Evidence: Child A, who has a diagnosed allergy to eggs and tree nuts, did not have a written allergy care plan in their record.

Plan of Correction: We will provide the form to the family and ask that they have the doctor complete it as soon as possible.

Description: Based on review of five staff records, the center did not obtain documentation that all staff have completed Virginia Preservice Training within the first 90 days of employment. Evidence: Staff D, with a start date of 7/1/19, did not have documentation of completing Virginia Preservice Training.

Plan of Correction: I will have them get me a copy as soon as possible.

Description: Based on review of five staff records, the center did not ensure that all staff have current certification in cardiopulmonary resuscitation (CPR). Evidence: Staff D, with a start date of 7/1/19, does not have documentation of current certification in CPR in their record.

Plan of Correction: I will have them provide me with a copy of their training today.
