Technical Assistance:
Discussed medication authorization form requirements, program lead qualifications, new employee orientation training, background checks, daily routine, proof of identity and subsidy requirements.

Comments:
Licensing inspector arrived at 9:50 am and departed at 1:45 pm to conduct a licensing inspection and a Subsidy Health and Safety inspection. At the time of the tour there was Forty-three children in care with 9 staff . Children were observed outside playing in multiple activity stations. The sample size was five staff records and six children records.

Description: Based on record review, the license did not ensure each child enrolled contained all required documents in the record.

Child #2's record was missing one emergency contact complete address.

Description: Based on 1 of 5 staff records reviewed and staff interview, the licensee did not ensure each staff record contained all required documentation.

Evidence:
Staff #3 did not have written documentation for orientation and documentation demonstrate that the individual possesses experience required by the job position.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the licensee did not ensure all areas and equipment of the center, inside and outside, were maintained in a clean, safe and operable condition.

Evidence:

- In the Pre-K classroom , there was white peeling paint on the window sill .

- In the Early preschool classroom , 1 of 2 bathrooms had yellow substances drops on the seat and toilet paper on the floor.

- In the two year old classroom, there was a brown chair with part of arm of the chair detaching, there was staples on the window sill and the floor had a wet substance possible slipping hazard around the table and there was areas of the room with dirt pushed on the side of the wall. In one of the bathrooms there was books and diaper on the floor and the other bathroom there was moveable dirt around the toilet ad the toilet lid was cracked. Staff #1 confirmed the observations in the classroom.

- In the infant classroom, there was a drawer that was broken and the corner of drawer is sharp could cause possible injury.

Description: Based on observation and interview the center failed to ensure that hazardous substances are kept in locked place using a safe locking method that prevents access by children.

Evidence: The kitchen was unlocked during the inspection and no staff were in the unlocked kitchen. Inside the kitchen there was disinfectant spray, soap and sanitizing tablets. The items were labeled with "caution". "keep out of reach of children" and "warning". Staff 1 confirmed and observed that the kitchen was unlocked and there were chemicals inside.

Description: Based on observation , the licensee did not ensure the rest rooms were equipped with toilet paper within reach of children.

Evidence:
In one of the two restrooms the two year old classroom , did not have toilet paper. Staff #1 observed and confirmed.

Description: Based on record review, in one of five staff records reviewed, the licensee did not deny continued employment to a staff member when the center did not have a central registry finding within 30 days of employment.

Evidence: Staff #1 confirmed that there was no documentation of a central registry finding for Staff #4 (date of hire 08/08/2019) and Staff #2 (Date of hire 09/20/2019) . Staff #1 confirmed the documentation was missing.

Description: Based on record review and staff interview, the center did not obtain out of state central registry findings for employees that have resided outside of the state of Virginia in the last five years.

Evidence: Staff # 2's record indicated that she has resides outside of Virginia, and has lived in another state within the last 5 years. Staff # 1 confirmed the a outs of state central registry forms has not been submitted.
