The following topics were discussed with the program director:
- daily playground inspections
- the metal stall walls in the restrooms are beginning to rust
- cords to the blinds in the school age classroom
- documentation if a parent does not bring in medication indicated on the allergy management plan
- additional information on injury records
An unannounced, mandated monitoring inspection was conducted today. The inspector arrived at 10:00 AM and departed at 12:15 PM. Morning activities in all classrooms were observed. The outdoor playground was inspected. Medication administration and injury records were reviewed. A sample size of five staff records and five children's records were reviewed.
An exit meeting was conducted with the program director prior to closure of the inspection.
Standard #: 22VAC40-185-70-A Description: Based upon review of five staff records and staff interview, the facility has not ensured that each staff record includes the name address and telephone number of a person to be contacted in an emergency.
1. The record provided for staff 4 did not include the address of the emergency contact person listed.
2. Staff 6 verified that the address was not indicated in the record.
Plan of Correction: The facility responded with the following:
The address was added to the record of staff 4 during the inspection. In the future we will ensure that the record of any newly hired staff person includes the address of the emergency contact person.
