Discussion was held in regards of resilient surfacing on playground and injury reports.
- Comments:

An unannounced monitoring inspection was conducted from approximately 2:00 pm to 5:00 pm. At the time of entrance, 21 children were in care with 5 staff members present. Children were observed during nap, snack time, engaged in group activities and outdoor play time. The site was clean, organized and contained a varied supply of developmentally appropriate materials. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, required posted documents, medications and the Emergency Preparedness Plan were reviewed. Areas of non-compliance are identified in the violation notice. Standard #: 22VAC40-185-240-A Description: Based on record review, not every staff supervising children completed recognizing child abuse and neglect training by the end of their first day of assuming job responsibilities as required .
Evidence:
1) The record for Staff #3 (start date 09/05/2019) did not contain documentation of having completed the recognizing child abuse and neglect training.Plan of Correction: Staff will complete the training by the end of this week.
Standard #: 22VAC40-191-60-C-2 Description: Based on document review and interview, not all staff supervising children had a central registry finding within 30 days of employment.
Evidence:
1) The record for Staff #3 (start date 09/05/2019) did not contain documentation of a central registry search or finding.
2) The director stated that she did not keep a copy of the form for the record.Plan of Correction: Home office have the documentation but we cannot access it at this time. We will provide the documentation to the inspector as soon as we have it.
