An unannounced renewal inspection was conducted on November 7, 2019 from approximately 9:40 am to 12:30 pm. Four staff were present with eleven children, ranging in age from infants through four years old. Additional staff were on site for administrative duties. Five children and five staff records were reviewed.

Standard #: 22VAC40-185-60-A Description: Based on review, facility did not ensure 2 of 5 children's records have documentation of all required information.
Evidence: The records for child #1 (start date August 13, 2019) and child #3 (start date October 14, 2019) did not have a complete address for two emergency contacts.Plan of Correction: Staff will get the information from parents.
Standard #: 22VAC40-185-70-A Description: Based on review, 2 of 5 staff records did not have documentation of all required information.
Evidence: The records for staff #4 (start date October 24, 2019) and staff #5 (September 11, 2019) did not have documentation of work experience required by the position of lead teacher.Plan of Correction: Administration will check back with references to verify work experience. Staff will be asked to complete a application and submit a resume.
