The following TA was provided: 22 VAC 40-185-350.D (ratios during designated rest and sleeping periods) - Ratios should not be doubled until the vast majority of children are settling down and resting.

The licensing inspector conducted an unannounced inspection on Friday, October 4, 2019 in response to a self-reported incident that the Department received on September 12, 2019 regarding supervision. The investigation was initiated on October 4, 2019 and completed on November 1, 2019. On 10/04/2019, the inspector was on site from 11:00am to approximately 12:27pm. Interviews were completed, observations were made in the center, and documentation was reviewed. Violations of the standards related to staffing were found during the inspection.
Please complete the `plan of correction? and `date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Standard #: 22VAC40-185-350-E-4 Description: Based on a review of documentation and interview on 10/04/2019, the center did not maintain the required staff-to-child ratio for children three years to the age of eligibility to attend public school, five years by September 30.
Evidence: 1) A review of the center's Child Supervision Record (CSR) revealed that a total of 13 children, ages 4-5 years, were present in the Pre-K classroom on 09/12/2019 at 12:30pm when the children went down for nap/rest time.
2) During interviews, staff reported that on 09/12/2019, Staff #1 was the only staff member present in the Pre-K classroom from approximately 2:20pm to 2:30pm when 13 children were getting up from nap/rest time. The required staff-to-child ratio was one staff member for every 10 children.Plan of Correction: Not available online. Contact Inspector for more information.
