Technical Assistance:
The following TA was provided: 22 VAC 40-185-350.D (ratios during designated rest and sleeping periods) - Ratios should not be doubled until the vast majority of children are settling down and resting.

Comments:
The licensing inspector conducted an unannounced inspection on Friday, October 4, 2019 in response to a self-reported incident that the Department received on September 12, 2019 regarding supervision. The investigation was initiated on October 4, 2019 and completed on November 1, 2019. On 10/04/2019, the inspector was on site from 11:00am to approximately 12:27pm. Interviews were completed, observations were made in the center, and documentation was reviewed. Violations of the standards related to staffing were found during the inspection.

Please complete the `plan of correction? and `date to be corrected? for each violation cited on the violation notice and return it to me within 5 business days from the date of receipt. Please specify how the deficient practice will be or has been corrected. Your plan of correction should contain: 1) steps to correct the noncompliance with the standards, 2) measures to prevent the noncompliance from occurring again, and 3) person(s) responsible for implementing each step and/or monitoring any preventative measure(s).

Violations:
Standard #: 22VAC40-185-350-E-4
Description: Based on a review of documentation and interview on 10/04/2019, the center did not maintain the required staff-to-child ratio for children three years to the age of eligibility to attend public school, five years by September 30.

Evidence: 1) A review of the center's Child Supervision Record (CSR) revealed that a total of 13 children, ages 4-5 years, were present in the Pre-K classroom on 09/12/2019 at 12:30pm when the children went down for nap/rest time.

2) During interviews, staff reported that on 09/12/2019, Staff #1 was the only staff member present in the Pre-K classroom from approximately 2:20pm to 2:30pm when 13 children were getting up from nap/rest time. The required staff-to-child ratio was one staff member for every 10 children.
