An unannounced renewal inspection was completed on 11/06/2019. There were 37 children, ages 3 months-4 years, and 7 staff present during the inspection. The inspector reviewed 5 children's record, 5 staff records, 1 agent record, and 4 medications during the inspection. Children were observed in the following activities: having breakfast, during free choice/center play, during lunch and during nap time. Proper hand washing, table sanitizing and diapering procedures were also observed. The inspector reviewed the emergency preparedness procedures, the updated injury prevention plan, transportation requirements, and emergency supplies during the inspection. The inspector discussed the playground and transportation with the staff. Standard #: 22VAC40-185-60-A Description: Based on record review and discussion with the staff, the facility failed to ensure that the separate record for each child enrolled shall contain all of the elements as required by the standards.
Evidence: The record for Child #2 did not contain documentation of child updates and confirmation of up-to-date information in the child's record as required by 22VAC40-185-420-E-3.Plan of Correction: All children's records will be reviewed to ensure that all are up-to-date.
Standard #: 22VAC40-185-540-E Description: Based on observations and discussion with the staff, the facility failed to ensure that the required nonmedical emergency supplies shall be on-site.
Evidence: There was not a working battery-operated radio in the school-aged building.Plan of Correction: A radio will be purchased today.
Standard #: 22VAC40-185-560-G Description: Based on observations, the facility failed to ensure that when food is brought from home the food shall be clearly dated and labeled in a way that identifies the owner.
Evidence: There was a plastic ziplock baggie of cereal and a juice drink in the cubbie area of the downstairs Pre-K room that was not clearly dated or labeled to identify the owner.Plan of Correction: No outside food from home will be allowed at the center.
Standard #: 22VAC40-191-60-C-2 Description: Based on record review and discussion with the staff, the facility failed to obtain a central finding within 30 days of employment.
Evidence: The record for Staff #5 had a documented hire date of 05/30/2018; the date on the central registry finding was 07/28/2018.Plan of Correction: A call will be made when findings are not received in 30 days.
