Comments:
An unannounced monitoring inspection was completed on 11/07/2019. There were 11 children, ages 2 years-5 years, and 3 staff present during the inspection. The inspector reviewed 5 children's record and 3 staff records during the inspection. The staff stated that medication is not being administered. Children were observed in the following activities: having breakfast, during free choice/center play, during learning time, in music and movement, going outside to play and during lunch. Proper hand washing and table sanitizing were also observed. The inspector reviewed the emergency supplies during the inspection. The inspector discussed subsidy requirements, restroom breaks and renovations to the basement with the staff. The inspector arrived for the inspection at 9:35 a.m. and departed at 12:30 p.m. If you have any questions, please contact Becky Forestier at 540-309-2835.

Description: Based on observations, record review and discussion, the vendor failed to maintain, in a way that is accessible to all staff who work with children, a current written list of all children's allergies, sensitivities, and dietary restrictions.

Evidence: The record for Child #4 and Child #5 contained documentation of diagnosed food allergies. There was not a written list of children's allergies that was accessible to staff. Staff #1 confirmed that there was not a list available.

Plan of Correction: The model form will be used to make a list.

Standard #: 22VAC40-665-520-B-11
Description: Based on record review and discussion, the vendor failed to obtain a written allergy care plan for each child with a diagnosed food allergy, to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
1. The record for Child #4 contained documentation of a diagnosed food allergy; there was not a written allergy care plan to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction available for Child #4.
2. The record for Child #5 contained documentation of a diagnosed food allergy; there was not a written allergy care plan to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction available for Child #5.

Plan of Correction: Parents will be provided a form with a deadline to return.
