REVISED VIOLATION NOTICE
A self-reported incident regarding sight and sound supervision of children was investigated on 10/28/2019. Based on evidence gathered, violations were cited and can be seen on the violation notice. Standard #: 22VAC40-185-40-E Description: Based on a review of the center's policies and procedures and video footage, it was determined that the licensee did not ensure that staff followed the center's policies and procedures required by the standards supervision of children in care.
Evidence:
1. The center's policies and procedures regarding supervision of children states that, "staff must account for all children in care at all times".
2. On October 24, 2019, at approximately 3:10pm staff #1 was observed lining the children up to go outside and marking something on a clipboard while they were lining up.
3. Staff #2 was observed filling water bottles, as the children exited the classroom door to go to the outside playground at approximately 3:16pm.
4. Child #1, (4 years old), was observed walking out of the bathroom into the empty classroom approximately 3:31pm.
5. Child #1 walked around the classroom, looked out the propped open door to the outside, then sat on the carpet.
6. At approximately 3:33pm, visitor #1 walked into the classroom, spoke with child #1 and left the classroom to bring the child to the front office.Plan of Correction: Since the event occurred,new policies and procedures have been implemented.
1. We do a ratio check every 30 minutes in each classroom, as well as check the roll call sheet to ensure compliance.
2. We have implemented a new policy that before any classroom transition occurs, staff must inform the front office and the manager on duty must observe the name to face, count the children compared to ipad count, and do a actutal sweep of the classroom.
3. Same applies to coming back in from playground.
These policies went in effect on October 24th, right after the incident happen, and we have been doing it everyday. There are 3 managers on duty, to ensure compliance.

Evidence:
