Technical Assistance:
Consultation provided on medication and monthly escape drills.

Comments:
An unannounced monitoring inspection was conducted today with the director, who was present with two additional staff and 10 children. The children had recently returned to the library from outside time and were engaged in homework/quiet activities. Six children ad four staff files were reviewed today as well as 7 medications for 4 children.
Arrival time: 4: 25 pm
Departure time: 5: 40 pm

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, it was determined that the outside play area was not maintained in a safe manner.
Evidence:
The ground covering is exposed and presents a tripping hazard on the playground

Plan of Correction: We will make sure that the exposed ground covering is cut and/or repaired.

Standard #: 22VAC40-185-330-B
Description: Based on observation, it was determined that there was insufficient resilient surfacing in the fall zones of the play equipment on the playground.
Evidence:
There was 1/4th of an inch of pea gravel directly underneath the 8 swings. The swings requires at least six inches of pea gravel.

Plan of Correction: We will rake the pea gravel into the fall zones of the play equipment.
