Comments:
An initial inspection was conducted today. No children are in care, thus those standards regarding the care and supervision of children were not reviewed. Written parent and staff policies were reviewed. The physical plant and playground space was measured and inspected. The facility approved space consists of 4 designated spaces for child care and an outdoor fenced in play space. There is also a foyer, one office, and a kitchen in the building. The, classroom capacity is calculated based on 35 square feet per child. Measurements of classrooms will allow the capacity of 39. The director's qualifications were also reviewed. Development. The outside play space is an open area with bikes/tricycles and small playhouses enclosed by fencing. There was one violation cited at this inspection which was discussed with the director at the conclusion of this inspection and is detailed in the violation notice. Any areas of standards related to direct service that were not determined during the initial inspection will be determined at the first unannounced inspection following the issuance of a license. All future inspections will be unannounced. The first unannounced inspection will be conducted within 60 days from the date the license is issued. Standard #: 22VAC40-185-270-A
Description: Based on observation it was determined the center did not ensure that all areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition.
Evidence:
1. In the Green room(3-5 year olds) there was 3 coaxial connectors protruding from plate covers and a loose phone/internet box on the wall at child height which presented potential injury hazards.
2. There were loose industrial staples hanging from the coat rack which presented potential injury hazards.
3. The changing tables in the restrooms rocked when pulled on and were not anchored which presented tip/crush hazards.
4. On the playground the landscaping borders around the perimeter of the designated play space had uneven ends which presented potential trip hazards.

Plan of Correction: All indoor repairs will be completed by 11/15/19. The playground will be repaired by 11/18/2019.
