An unannounced monitoring inspection was conducted between 8:00 am and 9:00 am. Upon arrival there were 23 children in care with 2 staff present. A sample of 4 children's record were reviewed. The center did not have any medications and the program director confirmed no children with food allergies were in care.

Standard #: 22VAC40-185-350-C Description: Based on observation and interview, the center did not ensure ratio requirements were met when in regularly ongoing mixed age groups.
Evidence:
The center has a before and after school program that includes a regularly mixed age grouping of children ranging ages 3 years- 12 years. When the inspector arrived around 8:00 am there were 23 children in care. Four of the children were preschool age, with the youngest child being 4 years-old. The ratio applicable to the youngest child is 1 staff per 10 children. The center had 3 children over the required ratio of staff. The program director confirmed that they were short staffed and were in the process of hiring.Plan of Correction: The following corrective action was provided by the director during the exit interview:
The third staff member recently left with little notice. The company is in the process of hiring another staff.

Based on observation, Using a harsh and stern tone, the inspector and program director heard Staff #1 tell a child in care,"Get in the circle!" She then said, "Little girl, do it again," daring her to step out of the circle. No other guidance or redirection was given.Plan of Correction: The following corrective action was provided by the director during the exit interview:
The director is aware that staff #1 does not allows guide the children's behavior in a positive way. The director will talk with the staff about other ways to redirect children.
Standard #: 22VAC40-185-540-D Description: Based on observation and interview, the center did not ensure all required emergency supplies were available.

The center did not have an emergency radio that worked without electricity or extra batteries.Plan of Correction: The following corrective was provided by the program director:
The director just purchased batteries and has misplaced them. If she is unable to locate the batteries she already bought, she will purchase new ones.

The director will add the shelter-in-place map and procedures to the evacuation map posted in the classroom.
