An unannounced monitoring inspection was conducted today. All rooms are in ratio during the inspection. Medications, emergency evacuation drills, shelter-in-place drills, first aid supplies, injury reports and labeled bottles are all in compliance.

Standard #: 22VAC40-185-60-A Description: Based on a review of records, one child's record does not contain all required information.
1 of 4 children's records reviewed was missing documentation of complete addresses for emergency contacts. (Child 1)
Plan of Correction: The parent will be asked to complete the address information.
Standard #: 22VAC40-185-280-B Description: Based on direct observation, hazardous materials were not stored locked in the Infant 1 room.
A bleach water spray bottle was on the counter, and the cabinet containing other hazardous materials was not locked.
Plan of Correction: Hazardous materials will be stored locked.
Standard #: 22VAC40-185-420-E-1 Description: Based on a review of documentation, infants time spent on their stomachs is not being recorded.
Time spent on stomachs is not recorded for to infants that require 30 minutes a day. (Children 1,2).
Plan of Correction: A meeting will be had with all infant team to discuss tummy time and how to record tummy time on the Ipad.
