Comments:
An unannounced Renewal Inspection was conducted today from 10:00am to 12:13pm. There were 84 (2s to school-age) directly supervised by 9 staff. The physical plant, 10 staff records, 7 children?s records, children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, learning as a group and reading out-loud, listening to stories, playing outside. Hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid, as well as DHO training. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-160-C
Description: Based on review, the facility did not ensure that at least every two years from the date of the first initial screening or testing, or more frequently if recommended, staff members shall obtain and submit results of a follow-up screening.
Evidence: Reviewed 10 staff records and found that Staff 8 did not have documentation of a subsequent screening in the last two years. The last documented screening was done on 1/1/17.

Plan of Correction: Staff 8s Tb Tine test was found in (their) other file and was put into the correct file. Staff 8 is a primary teacher in our private school.

Standard #: 22VAC40-185-210-A
Description: Based on review, the facility did not ensure that they had documentation of program leader qualifications.
Evidence: Reviewed 10 staff records and found that Staff 8 did not have documentation of program leader qualifications.

Plan of Correction: Staff 8 brought her transcripts to show that her degree is in education. Degree was in her file.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the facility did not ensure that areas and equipment of the center, inside and outside, shall me maintained in a clean, safe and operable condition.
Evidence: Observed a hole and torn siding at the rear of the building 2, accessible to children.

Plan of Correction: Our maintenance department will be getting the siding fixed.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility did not ensure that hazardous substances are kept in a locked place using a safe locking method that prevents access by children.
Evidence: Observed in JK-C, lice spray, marked keep out of reach of children in an unlocked cabinet; Observed in Building 2 and unlocked cabinet with Clorox, peroxide and fantastic cleaner in an unlocked cabinet accessible to children.

Plan of Correction: Lice spray was removed and the cabinet will remained locked at all times. Hallway closet will remained locked in our primary hallway.
