Comments:
An unannounced focused monitoring visit and facility tour was conducted from 11:11 am-2:30 pm. At the time of inspectors' arrival there were 33 infant to preschool aged children in care with 5 staff members. Additional staff members and the Program Director arrived while the inspector was present. Children were observed participating in free play, lunch, transitioning between activities, hand washing and diapering. A sample of 7 children's records and 7 staff records were reviewed. Noncompliance was identified in the areas of staffing and supervision, nutrition, and background checks and can be found on the violation notice. The areas of noncompliance were discussed with the program director during the exit meeting at the conclusion of this inspection.

Standard #: 22VAC40-185-350-E-1
Description: Based on observation and interview it was determined the center did not ensure that there was one staff member for every four children aged birth to 16 months.
Evidence:
1. Staff #6 was the only staff person present in the infant class from 11:11-11:20 am with 6 children aged 16 months and younger, where 1 staff per every 4 children was required.
2. Staff #6 confirmed that they were the only staff person in the class and the class was out of ratio. They further reported that staff #2 was "on her way" and that staff #3 "had been in the class but had left".

Standard #: 22VAC40-185-560-F-4
Description: Based on observation it was determined the center did not ensure that children three years of age and younger were not offered foods that are considered to be potential choking hazards.
Evidence:
1. Whole 1" round sections of bananas and 1" meatballs were served for lunch to children aged three and younger during this inspection presenting potential choking hazards.
2. Staff #8 stated that they "have been so focused on not serving hot dogs that we didn't think these foods were an issue".

Standard #: 22VAC40-191-60-B
Description: Based on record review and interview it was determined the center did not ensure that employee is not employed prior to completing a sworn statement.
Evidence:
1. There was no documentation of a completed sworn statement for staff #2(hired 10/17/19).
2. Staff #8 confirmed that this documentation was not available during this inspection.
3. Staff #2 was present and caring for children during this inspection.
