Comments:
An unannounced monitoring inspection was conducted on 11/14/2019 from 4:00 pm - 5:05 pm. At the time of entrance there were 45 school age children in care with three staff. Children were observed during afternoon program time which included snack and organized games in the gym. Records were reviewed for nine children in care. Medication and emergency supplies also reviewed.
Violations were observed in four parts of the CDC standards and are listed on the violation notice.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that children's records contain all of the required elements.

Evidence:
1. The enrollment record for child 1 lacked a work phone number for each parent's place of employment.
a. The record indicated that each parent was employed.

Plan of Correction: Asked parent for work phone number.

Standard #: 22VAC40-185-270-F
Description: Based on observation and interview, the center to ensure that the equipment of the center shall include an in-service, nonpay telephone.

Evidence:
1. The program director stated that the facility did not have an in-service telephone.
a. The program director stated that there had been a working telephone but it had ceased working "a long time ago."

Plan of Correction: Site has received a cell phone for use at after-school.

Standard #: 22VAC40-185-510-E
Description: Based on observation, the center failed to ensure that medication shall be labeled with the child's name.

Evidence:
One container of an over-the-counter emergency medication was not labeled with the child's name.

Plan of Correction: Labeled medicine with child's name.

Standard #: 22VAC40-185-560-F
Description: Based on menu review and observation the center failed to ensure that when snacks are provided centers shall follow the most recent, age-appropriate nutritional requirements of a recognized authority such as the Child and Adult Care Food Program of the United States Department of Agriculture (USDA).
*The posted menu for the current one-week period shall list the foods served.

Evidence:
1. Fig bars are listed on the menu as snack for each Wednesday in November.
a. A child was provided with a fig bar for snack during the inspection because he did not want the scheduled snack of fruit cup and pretzels.
b. Fig bars do not meet USDA guidelines of the required serving of 3/4 cup fruit and 1 slice or 1/2 cup of grain for children ages 6 - 12 years. Additionally, the fig bars served do not meet current USDA guidelines due to the high sugar content.
2. The posted menu does not list the snack to be served for each Friday in November.
a. The menu indicates the shack for each Friday as "Kids Choice."

Plan of Correction: Staff followed directions on sanitizer. Will let tables air dry going forward.

Standard #: 22VAC40-185-560-K
Description: Based on observation, the center failed to ensure that tables used for feeding shall be sanitized after each use. *The definition of sanitized means that the surface is sprayed with the sanitizing solution and allowed to air dry.

Evidence:
The inspector observed staff sanitize two tables after snack. Staff used a paper towel to immediately wipe down the tables after spraying the sanitizing solution.

Plan of Correction: We will no longer serve fig bars for snack & provide a snack on Friday's.
