Comments:
The inspector conducted an unannounced mandated inspection from 9:40 a.m. until 11:30. There were 8 children in care with two staff supervising. Another staff arrived at 11:00. Children were observed at circle time, washing hands, eating breakfast, and playing outside. Five children's records and six staff records were reviewed. Two violations had been corrected. No medications were being administered. An acknowledgement form was signed by the staff. Violations are identified on the violation notice. Standard #: 22VAC40-185-160-C
Description: Based on record review, the center did not have repeat TB screenings on all staff.
Evidence:
1. The inspector reviewed six staff records. One of six staff (#3) did not have a repeat TB screening. The last one expired 09/28/2017.

Plan of Correction: Staff will get the repeat TB screening.

Standard #: 22VAC40-185-200-A
Description: Based on interview, the center did not have a qualified program director or a qualified back-up program director who meets one of the director qualifications at least 50% of the center's hours of operation.
Evidence:
1. The inspector interviewed the program director by phone who stated she was not there 50% of the time.

Plan of Correction: The director stated she is currently getting rehabilitation services for an injury and plans to be back after Thanksgiving. She has been keeping in touch by phone and is at the center when able.

Standard #: 22VAC40-185-270-A
Description: Based on observation, the center had equipment outside and inside that was no maintained in a clean and safe condition.
Evidence:
1. The inspector observed peeling paint on the floor in the large room by the mats/door and also near the wall as you enter the door to the left.
2. The inspector observed play equipment outside that had rust: the train (on the bottom), the orange bench with steering wheels.

Plan of Correction: The floor will be repainted or a rug bought to cover the area.
The playground equipment was painted.

Description: Based on observation, the center did not keep all hazardous substances locked.
Evidence:
1. The inspector observed an bottle of "spray nine" disinfectant sitting in the office unlocked (door was open).

Plan of Correction: The director spoke with all the staff about the importance of keeping all hazardous substances locked.

Standard #: 22VAC40-185-330-B
Description: Based on observation and measurement, the center did not maintain the required depth of mulch on the playground.
Evidence:
1. The inspector observed and measured 1-2 inches of mulch surrounding the climber, bouncing car, 4 set bouncer and teeter-totter. There was no mulch at the end of the slide.

Plan of Correction: Mulch has been spread to the required depth.

Standard #: 22VAC40-185-330-C
Description: Based on observation, ground supports were not covered with materials that protect children from injury.
Evidence:
1. The inspector observed 2 ground supports exposed under the bouncing car.

Plan of Correction: They have been covered with mulch.
