Comments:
An unannounced renewal inspection was conducted from 9:25am-1:30pm. At the time of entrance,57 children were in care with 20 staff members present. Children were observed playing in centers, coloring, during diaper changes, hand washing and outdoor play time. Infants were observed being fed, held, sleeping, playing and during tummy time. The site was clean, organized and contained an abundant supply of developmentally appropriate materials. Interactions between the children and staff were positive. A selection of staff and children records, the physical space, emergency supplies, evacuation drills, attendance records, injury reports, medications and the Emergency Preparedness Plan were reviewed. . Standard #: 22VAC40-185-270-A
Description: Based on observation, areas and equipment of the center inside and outside were not maintained in a safe and operable condition.
Evidence:
1. A blue picnic table on the playground had rust on the bottom of legs and the paint was chipping near the bottom of the table.
2. The children's play sink was missing the faucet and had a bolt sticking up where the piece was broken off.

Plan of Correction: The maintenance guy is going to treat, fix and paint the picnic table.
The maintenance guy removed the broken piece and bolt and will replace the part.

Standard #: 22VAC40-185-500-B
Description: Based on observation, the center did not meet all diaper changing pad requirements.
Evidence:
1. The diaper changing pad had approximately 6 cracks on the surface of the pad and was not a nonabsorbent surface.

Plan of Correction: I have already ordered a replacement for the pad. It was ordered on 11/12/2019.

Description: Based on observation, the findings of the most recent inspection of the facility was not posted.
Evidence:
1. The inspection dated 9/24/2019 was underneath the Arlington inspection document in a wall mount sign holder and was not in view for the public..

Plan of Correction: I will post todays inspection and make sure it is in view.

Description: Based on observation and interview, the center failed to place a copy of the Notice of the Commissioner's intent to take any actions enumerated in subdivisions B 1 through B 7 of ? 63.2-1709.2 of the Code of Virginia.in a prominent place at each public entrance of the licensed premises to advise consumers of serious or persistent violations.
Evidence:
1. The Notice of Intent dated 10/28/2019 was not posted at the Center.
2. Staff A confirmed the notice of Intent dated 10/28/2019 was not posted.

Plan of Correction: I posted the notice for the parents to be able see.
