Comments:
An unannounced monitoring inspection was conducted today, November 14, 2019 to determine the centers compliance with licensing standards. The inspection was initiated at 3:20 pm and concluded at 5:30 pm. The inspector observed children arriving from the public schools. Children were checked in on the attendance sheet and then played with blocks, cards and looked at books. A total of 54 school age children were in care with six staff. Later children divided into groups by age where they worked on homework, painted pictures, and played a game of four corners in the gym. Snack time was also observed with hand washing being conducted. Today's snack consisted of chex-mix and pears. All areas used for child care were inspected. The facility has age appropriate materials and supplies. The facility had the following required information posted: license, last inspection notice, emergency numbers, menu, daily schedule, evacuation map/ procedures, and various parent information. The first aid kit and non-medical emergency supplies were inspected and found complete. Staff emergency information was reviewed on site. Staff records were review at the YMCA Downtown Administrative office on September 25, 2019. The inspector interacted with staff and children in care.

Five children's records were reviewed along with emergency contact information for six staff.
Medications, medication authorizations and allergy care plans were reviewed.
The most recent evacuation drill was conducted on 10/09/2019. The two required shelter-in-place drills have not yet been conducted. They will need to be completed by the end of the year. A lock-down drill was documented on 06/12/2019.

Violations:
Standard #: 22VAC40-185-280-B
Description: Based on observation and interview, the center did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides were kept in a locked place using a safe locking method that prevents access by children.

Evidence:

1. A bottle of cleanser/ sanitizer was observed unlocked on the shelf in the kitchen.
2. Two containers of lighter fluid were observed unlocked, on a shelf near the floor of the kitchen.
3. The kitchen is located off of the main child care classroom. The door was open and staff stated children used the hand washing sink in the kitchen for washing their hands.

Plan of Correction: Per the director: The bottles will be removed and placed in a locked area.

Standard #: 22VAC40-185-350-E-5
Description: Based on observation and interview, the center did not maintain a ratio of one staff for every 18 children for children from the age of eligibility to attend public school through eight years.

Evidence:

1. The licensing inspector observed one staff supervising 21 children, ages five years through eight years, in the gym.
2. The director acknowledged there was one staff with 21 children in the gym.

Plan of Correction: Per the regional director: We will make sure staff are where they should be. We will also have additional staff on site to cover in areas when needed. A staff member was moved to the group of children in the gym to meet the required ratio while the inspector was on site.

Standard #: 22VAC40-185-510-A
Description: Based on review of three medications, authorizations and medication administration logs, the center did not obtain written parent authorization before administring prescription and nonprescription medications.

Evidence:

The record of child #1, did not contain written parental authorization for a medication that was administered on 11/08/2019, 11/11/2019, 11/12/2019, 11/13/2019 and 11/14/2019. The record did contain authorization from the child's physician.

Plan of Correction: Per the director: I will request that the parent complete the authorization form today at pick up time for the record.
