An investigation was initiated on 11/19/19 in response to a self-reported incident that occurred on 11/14/19 regarding behavior guidance. An unannounced on-site inspection was conducted on 11/21/19 and interviews with staff were conducted. Records, documentation and video footage related to the incident were also reviewed. Based on information gathered during the investigation, the incident is determined to be not valid. No violations were cited as a result of the self-reported incident.
