Technical Assistance:
Discussed sight and sound supervision on the playground with Staff #2.
Discussed with staff #1 about kitchen supplies and mixed age group ratios.

Comments:
An unannounced monitoring inspection was conducted on November 07, 2019. The licensing inspector arrived at 3:10pm and departed at 6:10 pm. At the time of the tour there were 37 preschool children present with four staff members. There was 32 school-age children with two staff . Children were observed eating snack and playing outdoors. The sample size was five children's records, five staff records and four medications.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

The following was observed in the Preschool Area:

In the girls restroom grout pipe fitting rings on two of the two toilets was rusted. Next to to the sink there was approximately a two inch hole the sides were rough.

In the boys restroom, there was peeling and bubbling paint behind two of two toilets. The metal paper towel dispenser and a electrical outlet cover plate was rusted. Next to to the sink there was approximately a two inch hole the sides were rough . There was standing water behind the toilet. Staff #1 stated she was not sure where it came from or it could be urine. The floor in the bathroom was wet and slippery. Staff #1 observed the bathroom physical plant concerns.

The following was observed in the school age area:
In the multipurpose room, there was multiple areas with blue peeling paint and shelf for belongings were peeling.

Outside walking to the playground, there were boards that were loose, deteriorating, splintering and cracking.

Description: Based on observation , the licensee did not ensure on all electrical outlet there is protective covers.

Evidence:
There were four electrical outlets without protective covers. Staff #1 stated that there was slides that should cover the outlets but there not working properly.
