63.2(17) License & Registration Procedures

Technical Assistance:
Discussed the importance of obtaining background check results as required. Consultation provided regarding the diapering area.

Comments:
An unannounced non-mandated inspection was conducted this date. A total of 52 children with 14 staff were present in the classrooms. The purpose of this inspection was to evaluate compliance during the first two months of operation. Observation of all 5 classrooms, playground, bathrooms and diapering area was made. Children and staff files were reviewed. Medication and authorizations were reviewed. Emergency drills were documented monthly. Some violations found today. Standard #: 22VAC40-185-70-A
Description: Based on staff record review, two staff records were incomplete.
Evidence: Staff #1's record does not maintain documentation of Director qualifications. Staff #3 was hired on 8/19/19, two references dated 8/22/19 were obtained for this staff after the hire date.

Plan of Correction: Copies of Director?s transcripts have been requested and will be filed upon receipt. Contacting references will be done following school procedures and as part of the pre-hiring process.

Administration team has implemented a system to conduct a 2 person review of documentation prior to mailing to ensure forms are not returned due to incompleteness and/or clerical error.

Description: Based on observation of all five classrooms, one area in one classroom was found not maintained in a clean condition.
Evidence: Classroom #7 counter located at the sink had areas of rust from a metal soap container.

Plan of Correction: Metal soap container removed.

Standard #: 22VAC40-185-330-A
Description: Based on observation of the newly installed playground, one area of the playground was hazardous.
Evidence: The space between the wooden playground border and the iron fence presents a hazard for children's feet/ankles to fall into this area when playing on the playground.

Plan of Correction: Facilities team has been contacted and the hazardous area will be corrected.

Standard #: 22VAC40-185-430-B
Description: Based on staff interviews and observation of the playground, equipment was not age and stage appropriate for all children.
Evidence: The newly installed playground equipment is designated for children ages 6 to 23 months. The center is licensed for children up to age 12 years. Children over the age of 23 months were observed on the playground equipment.

Plan of Correction: New playground manufacturer confirmed the equipment conforms to ASTM standards for both 6-23m and 24m+, meets safety standards for both ages and was specifically designed to have sufficient activities to be appropriate for both age groups.

Description: Based on staff record review, Central Registry search results were not received within 30 days of employment.
Evidence: Staff #3 and #4 were hired on 8/19/19. Central Registry results were dated 11/4/19.

Plan of Correction: Administration team has implemented a system to conduct a 2 person review of documentation prior to mailing to ensure forms are not returned due to incompleteness and/or clerical error.

Standard #: 63.2(17)-1721.1-B-2
Description: Based on staff record review, background checks were not obtained prior to first day of employment.
Evidence: Staff #2 was hired on 8/12/19, fingerprint search results were dated 8/13/19. Staff #3 was hired on 8/19/19, fingerprint search results were dated 8/26/19. Staff #4 was hired on 8/19/19, fingerprint search results were dated 8/20/19.

Plan of Correction: Moving forward, new staff members will not be permitted to begin employee orientation until fingerprinting results are obtained. Finger printing appointment will be scheduled immediately following acceptance of employment offer.

Description: Based on review of staff records, one out of state Central Registry search was not conducted.
Evidence: Staff #4 documented on their Sworn Statement that they resided out of state during the past five years. No out of state Central Registry search was completed.

Plan of Correction: We will request this search immediately and forward the results to the licensing inspector within 10 working days.
Moving forward, new staff members will not be permitted to begin employee orientation until fingerprinting results are obtained. Finger printing appointment will be scheduled immediately following acceptance of employment offer.
