Technical Assistance:
Discussed safe sleep procedures , labeling cribs, resilient surface on the playground and menu.

Comments:
An unannounced renewal inspection was conducted on June 5, 2019 from approximately 8:25 am- 11:15 am. At arrival to the facility, there were 28 children with five staff members. The children were observed eating breakfast, singing songs, doing music and movement, transitioning into classrooms and playing outside. Licensing Inspector reviewed supervision, programs, activities, equipment, indoor and outdoor areas, children and staff records, emergency evacuation and shelter-in-place drill documentation, fire and health inspection reports, emergency supplies and required postings. Five children's records and seven staff records were reviewed. The exit interview was conducted with the Owner.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation and inspection of the facility, the licensee did not ensure that areas and equipment of the center, inside and outside, shall be maintained in a clean, safe and operable condition. Unsafe conditions shall include, but not be limited to, splintered, cracked or otherwise deteriorating wood; chipped or peeling paint; visible cracks, bending or warping, rusting or breakage of any equipment; head entrapment hazards; and protruding nails, bolts or other components that could entangle clothing or snag skin.

Evidence: The owner verified the following conditions of the outside and inside area:

1. Seven of the two picnic tables had cracks. One picnic had a crack with sharp edges on the corner of the picnic table bench. The second picnic table had a three inch crack with pinching points on the bench and a two inch cracked on the table.

2. The white paint was peeling on the exterior door entering the Infant/School Age building.

3. There was a approximately one inch hole with sharp edges in the boys bathroom door in the preschool building.

Plan of Correction: Tables will be removed. The door will be painted or replaced. The boys bathroom door will be replaced.

Standard #: 22VAC40-191-60-C-2
Description: Based on 1 of 7 records reviewed and staff interviews the licensee did not ensure each staff has central registry findings with in 30 days of employment.

Evidence: Staff #3 ( hire date 7/10/2019) did not have central registry results with in 30 days of employment.

Plan of Correction: The owner understood that the central registry was included in the fingerprints. Received clarification in October and submitted the Central Registry November 4, 2019. Spoken with central registry today December 5, 2019 and was told they are working on November now. And should have the Central registry results within a couple of days.

Standard #: 63.2(17)-1720.1-A
Description: Based on record review, licensee did not ensure all employees and staff will have updated central registry every 5 years.

Evidence:
Staff #4 most recent central registry was completed September 14, 2014 and staff #5 most recent central registry was completed September 14, 2014. The central registry was more than 5 years old.

Plan of Correction: The owner understood that the central registry was included in the fingerprints. Received clarification in October and submitted the Central Registry November 4, 2019. Spoken with central registry today December 5, 2019 and was told they are working on November now. And should have the Central registry results within a couple of days.
