Comments:
The center self reported an incident involving a preschooler being left unsupervised in a classroom. Interviews and a review of record keeping was conducted and there was one violations from this investigation.

Violations:
Standard #: 22VAC40-185-340-F
Description: Based on interviews ,it was determined that children under 10 years of age were not always within sight and sound supervision of staff.
Evidence:
On 9/27/19 at approximately 9:15 am, a staff observed child A, 3 years of age sitting alone in classroom # 108 doing work at the table. The child was returned to her assigned classroom #107. The teachers in classroom 107 reported that Child A went into the Jack and Jill bathroom approximately 3 minutes prior. Child A exited the bathroom into classroom #108 instead of classroom #107.

Plan of Correction: Both teachers will be given until the end of October 2019 to complete the VA DSS online Pre-Service training and submit their certificates to the office.
We have placed childproof safety locks on the outside of each of the Jack and Jill style bathrooms in the primary wing. All teachers have been instructed that whenever they are leaving their classroom with their children that they will lock the outside bathroom latch that leads into their empty classroom so no children can enter the room in this manner when no teachers are in the room.
