Comments:
The licensing inspector conducted an unannounced monitoring inspection on Friday, December 16, 2019 between 11:40 am to 2:30 pm to determine the center's compliance with licensing standards. During this inspection, initially there were 28 children present with 7 staff including the center's director and assistant director The licensing inspector observed the children in care participating in a variety of activities to include participating in circle time reading, completing bathroom breaks, engaged in active play on carpet, transitioning into napping, and napping. The first aid kit and emergency supplies, posted emergency contacts, emergency drills, menu, required postings were reviewed by the inspector. During this inspection, there were six children's records and five staff records reviewed. The center's bathrooms, classroom, kitchen, and transportation were inspected. Violations:
Standard #: 22VAC40-185-160-C
Description: Based on review of staff record, the center did not ensure all staff resubmit TB test results every 2 years.

Evidence: In Staff#1's record (date of hire: 2/22/16) the last dated TB was 10/24/2017.

Plan of Correction: The center obtain documentation from staff but it did not specify TB results. The center director will request further documentation from the staff today.

Standard #: 22VAC40-185-60-A
Description: Based on review of children's record and interview with staff, the center did not ensure each child record contains the following:

Evidence: In Child#1's record (start date 10/23/19), the address for the second designated person to call in an emergency if a parent cannot be reached was incomplete.

Plan of Correction: The center director will ensure information is completed on 12/06/19. The center will ensure all children information is completed in records.

Standard #: 22VAC40-185-270-A
Description: Based on observations at the center, the center did not ensure all areas and equipment of of the center were maintained in a clean, safe, and operable condition.

Evidence: 1. In the Toddlers classroom, there was a multi-color cushion located on the floor with multiple torn areas with the exterior material exposed causing a safe hazard for the ages and stages of development of the children in care.
2. Exhibit #3 photograph taken of cushion condition during the inspection.

Plan of Correction: The center will replace cushion and order new item today.

Standard #: 22VAC40-185-280-B
Description: Based on observations, the center did not ensure all hazardous substances were kept in a locked place using a safe locking method that prevents access by children.

Evidence: During the inspection on December 6, 2019, the following observations were made:
1. The center's front bathroom was observed unlocked containing window cleaner with the labels "keep out of reach of children" and "caution" which was accessible to the children in care.
2. The center's kitchen bathroom door was observed opened with the cabinet unlocked containing disinfectant sprays with the labels"keep out of reach of children" and "warning" accessible to the children in care.
3. In the infant room, there were two bottles of disinfectant sprays with the labels "keep out of reach of children" and "caution" located on the top of a storage rack.
4. Exhibit #1 and #2 photographs of the unlocked cabinets and cleaner supplies taken during the inspection.

Plan of Correction: The center will ensure all hazardous substances are locked and locking methods are being applied by staff at all time.

Standard #: 22VAC40-185-350-E-1
Description: Based on observations, review of children's record, and interview with staff, the center did not ensure to follow the staff to children ratios as required,

Evidence: 1. In the Toddlers classroom, there were two staff with 9 children in care, the youngest child in the group being Child#2 (14 months). The staff to children ratio for birth to 16 months; one staff to 4 children.
2. Interview with staff confirmed that Child#2 was the youngest in the group.
3. A review of Child#2 record verified age.

Plan of Correction: Corrected on 12/6/19. The center will ensure to follow staff to children ratios for each group.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview with staff, the center did not ensure diapering surface shall be used only for diapering or cleaning children.

Evidence: In the Toddlers classroom, there were artwork activities observed on the diapering station during the inspection. Interview with staff confirmed the diapering station is being used by staff for the children in care.

Plan of Correction: The center will ensure the diapering stations are used for diapering and cleaning children only. Staff will reminded of center procedures.
