An unannounced subsidy health and safety inspection was conducted on 12/05/2019 from 1:10 pm - 3:30 pm. There were a total of 35 children in care with seven staff. The children ranged in age from 12 months - five years. Children were observed during nap and afternoon program time.
Records reviewed for nine children and three staff.
Medication, emergency supplies and allergy documentation additionally reviewed.

Standard #: 22VAC40-665-540-A Description: Based on record review, the vendor failed to ensure that the TB screening documentation shall have been completed within the last 30 calendar days of the date of employment.
Evidence:
1. The TB screening for staff 1 was completed on 09/26/2019.
a. Staff 1 has a hire date of 11/05/2019.Plan of Correction: Staff 1 obtained an updated TB screening on 12/09/2019.
