Comments:
An unannounced monitoring inspection was conducted today from approximately 11:50am-1:20pm and on 11/10/2019 from approximately 11am-1:30pm. There were 32 children in the direct care and supervision of 13 staff. A sample of 5 children?s records and 5 staff records were reviewed. The children were observed napping, and engaged with staff. The center will be closed 12/09/2018 for visits. 5 new staff have been hired since the last inspection. Areas of non-compliance are identified in this report. Description: Based on interview, the center did not have documentation of liability insurance.

Evidence:
The staff could not provider proof of current liability insurance.

Plan of Correction: I have the plan and it will be sent to the location today.

Description: Based on interview, the center did not develop written procedures for injury prevention.

Evidence:
Staff A stated the injury prevention plan may be at the other location.

Plan of Correction: Plan was sent on the 4th.

Description: Based on record review, the record for a child in care was missing required information.

Evidence:
Child A was missing the employer and employer phone number of both parents.

Plan of Correction: I will get that information today.

Description: Based on interview, staff records were missing information to be kept at the location.

Staff were missing the following required information to be kept at the location:

Address
Verification of age requirement
Job title,
Date of employment
and name, address and telephone number of a person to be notified in an emergency.

Plan of Correction: I will make a copy of a revised form with required information.

Evidence:
Disinfectant day was found in the Pathfinders room in a unlocked cabinet.

Description: Based on record review, an employee of a licensed facility did not have central registry findings within 30 days of employment.

Evidence:
Staff B, hired 10/2019, did not have central registry findings on file.
