Technical Assistance:

Provided consultation on ensuring that locks used on cabinets actually work, and prevent access by children. Discussed safe sleep practices.

Comments:

An unannounced monitoring inspection was conducted from 9:40 am ? 12:40 pm with the center director and assistant director. There were 24 children, ranging from six months to four years of age, present with five staff supervising. The children were observed having free play, napping (infants), and participating in teacher lead circle time activities. Four children's records and four staff records were reviewed. One medication/authorization form was reviewed and in compliance. The emergency drill log was observed and in compliance. The playground was not observed due to the weather. Standard #:	22VAC40-185-140-A
Description:	Based on a review of four children's records, the center did not obtain documentation that each child has received a physical examination prior to the child's first attendance or within one month after attendance. Evidence: the record for child #2 (date of first attendance: 8/26/19) and child #3 (date of first attendance: 8/6/19) did not have documentation of a physical examination.
Plan of Correction:	Will ask the parents to provide the missing documentation.
Standard #:	22VAC40-185-160-C
Description:	Based on review of four staff records, the center failed to have a repeat tuberculosis (TB) test or screening for each staff at least every two years from the date of the previous screening. Evidence: the last documented TB test or screening for staff #3 was dated: 8/29/17.
Plan of Correction:	Staff #3 will be asked to obtain an updated TB test/screening.
Standard #:	22VAC40-185-260-A
Description:	Based on a review of documentation, the center did not have an annual fire inspection. Evidence: The most recent fire inspection report on file was dated 11/27/18.
Plan of Correction:	The center had a fire inspection last month. Will follow up with the fire marshall to obtain the written inspection report.
Standard #:	22VAC40-185-260-B
Description:	Based on a review of documentation, and an interview with the director, the center did not have an annual health inspection. Evidence: The most recent health inspection report on file was dated 3/27/18.
Plan of Correction:	Will continue to follow up with health department to get a health inspection scheduled.
Standard #:	22VAC40-185-280-B
Description:	Based on observation, not all hazardous substances were kept in a locked place. Evidence: there was a bottle of restroom cleaner on top of a shelf in the two's classroom.
Plan of Correction:	Cleaner was moved to a locked storage cabinet during inspection.
