Thank you for your assistance during this unannounced inspection. This on-site visit was conducted on October 10, 2019 as a result of a complaint that was received from the Valley Licensing Office on September 25, 2019 regarding the allegation of supervision. Interviews were conducted and documentation reviewed. The information gathered during this complaint supports the allegation and the complaint is valid.

Standard #: 22VAC40-185-340-A Complaint related: No Description: . Based on documentation and interview, the center failed to ensure the care and protection of one child on care.
Evidence:
1. Staff #1 stated she put Child #1 down for nap.
2. Staff #1 stated she had to move Child #1?s cot to the other side of the room because another child was bothering him.
3. Staff #1 stated she heard Child #1 begin to cry and went to check on him. Staff #1 patted his back and Child #1 went back to sleep.
4. Staff #2 arrived to relieve Staff #1 for lunch.
5. Staff #2 heard Child #1 crying and went to console him.
6. Staff #2 stated saw a bruise on his eye and asked Staff #1 what happened. Staff #1 stated Staff #2 said, ?I have no idea.?Plan of Correction: Remind Staff when a child wakes up crying they should check them to assure the child hasn't hurt themselves. During nap time don't assume anything.
