63.2(17) License & Registration Procedures

Technical Assistance:
Per our conversation, authorizations for topical ointments are required for any over the counter topical ointments and sunscreens.

Comments:
An unannounced Monitoring inspection was conducted this date. A total of 77 children with 9 staff present during the inspection. Walk thru of all occupied classrooms was made. The children were observed sleeping on their cots. Staff were observed sitting beside some children encouraging sleep. In the infant room, diapering procedures were observed along with hand washing procedures. Infants were experiencing tummy time and some were being held while being bottle fed. The classrooms were decorated with artwork consistent with the curriculum for the week. All about Me profiles of children were hanging up along with the supplies to make construction paper snowman were laid out for the activity. A total of 5 children's record and 5 staff records were reviewed. This inspection began at approximately 12:10pm and ended at 2:30pm. Thank you for your continued cooperation. Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation of the center, one area was found not maintained in a safe condition.
Evidence: The fencing located off the PreK classroom is bent with sharp edges, some edges beginning to rust. The gate located on this fencing has a gap between the gate and the pole which presents a head entrapment hazard.

Plan of Correction: We will have our maintenance representative to come out and address it towards the end of the week.

Standard #: 22VAC40-185-500-B
Description: Based on observation of the Infant 2 class, the diapering surface was no longer absorbent.
Evidence: The diaper mat located in the Infant 2 classroom, has a crack in it and is no longer absorbent.

Plan of Correction: We will obtain a new mat.

Standard #: 22VAC40-185-520-A
Description: Based on review of the topical ointments in the Infant class, one topical ointment was kept beyond the expiration date.
Evidence: One topical ointment was found with an expiration date of January 2017.

Plan of Correction: We will dispose of this medication.

Standard #: 63.2(17)-1720.1-B-3
Description: Based on staff record review, one background search was not conducted for one staff.
Evidence: Staff #1 indicated on their Sworn Statement dated 10/19 that they have lived in NJ within the past 5 years. A central registry search was not requested from the state of NJ.

Plan of Correction: We contacted NJ during the inspection and began the process to request the background search. One results are returned, we will forward to the Licensing Inspector
