Comments:
An unannounced monitoring inspection was conducted on 12/11/2019 from 12:30 pm - 1:30 pm. At the time of entrance there were 36 children in care with five staff. The children ranged in age from infant - four years. Nap time was observed.
Records were reviewed for eight children and four new staff. Medication, injury documentation and emergency supplies additionally reviewed.
The violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-270-A
Description: Based on observation, the center failed to ensure that equipment of the center shall be maintained in a safe operation.

Evidence:
1. The metal seat was missing from the play car on the playground. A sharp edge was left on the metal attachment bar where the seat used to be.

Plan of Correction: The PD will contact the company for a replacement seat. But for time being the metal piece will be covered with tape.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the center failed to ensure that hazardous substances such as cleaning materials shall be kept in a locked place using a safe locking method that prevents access by children.

Evidence:
1. The child proof latch on the cabinet under the diapering changing table no longer worked. This resulted in the cabinet being easily opened.
a. A bottle of sanitizer was observed inside of this cabinet.

Plan of Correction: The child proof latch was replaced. The staff was reminded when something is no longer in working conditions to notify the PD.

Standard #: 22VAC40-185-440-J
Description: Based on observation, the center failed to ensure that there shall be at least thirty inches of space between service sides of occupied cribs and other furniture where that space is the walkway for staff to gain access to any occupied crib.

Evidence:
1. Three occupied cribs lacked 30 inches of space on the service side.
a. The service side for crib 1 measured 15 inches.
b. The service side for crib 2 measured 22 inches.
c. The service side for crib 3 measured 18 inches.

Plan of Correction: The cribs were re-positioned with the correct spaces between them. The staff will be reminded that the cribs should always have the proper spacing.
