Thank you for your assistance during this unannounced monitoring inspection. This on-site visit was conducted on September 25, 2019 as a result of a self-reported incident made to the Valley Licensing Office on September 19, 2019. Interviews were conducted with two staff and documentation was reviewed. The information gathered does not support any violations of licensing standards.
