An unannounced Renewal Inspection was conducted on December 13, 2019. The inspection was from approximately 11:50 am until 2:00 pm. There were nine staff present with fifty children. The sample size of records reviewed included six children's records and eight staff records. Seven medications were reviewed and two center vehicles were inspected. Violations from the previous inspection were reviewed. The results of the inspection were reviewed and verified with the center director during the exit interview. Areas of non-compliance are identified on the violation notice.

Standard #: 22VAC40-185-280-B Description: Based on observation and staff interviews, the licensee did not ensure hazardous substances such as cleaning materials, insecticides, and pesticides shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: In the yellow room the cabinet near the diaper changing station does not have a lock and there were hazardous chemicals, disinfectant, cleaner, bleach/water solution labeled "keep out of reach of children" and at least one other statement "caution", "flammable" and "warning".Plan of Correction: Replace lock on cabinet.
Standard #: 22VAC40-185-510-J Description: Based on observation and staff interviews, the licensee did not ensure that all medication, except for those prescriptions designated otherwise by written physician's orders, including refrigerated medication and staff's personal medication, shall be kept in a locked place using a safe locking method that prevents access by children.
Evidence: On the "white van" in the unlocked first aid kit, there were 4 pain reliever medications and 4 anti-acid medications.Plan of Correction: Remove all such medications from the 1st aid kit.
