Comments:

A Subsidy Health and Safety inspection was conducted on December 12, 2019, to review supplemental health and safety requirements. This on site visit was from 9:20 a.m. through 12:30 p.m. There were nineteen (19) children in the care of four staff in three classrooms. Five children?s and six staff records were reviewed and updated. One medication and authorization paperwork was reviewed. A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice. Areas observed were: free choice activity, books, hand washing/diapering procedures, lunch, and nap. Interactions between the children and staff were observed as appropriate and the children seem comfortable with the daily routine and schedule. Standard #:	22VAC40-665-540-A
Description:	Based on review of staff records, the center failed to obtain a tuberculosis screening prior to staff coming into contact with children or within the last 30 calendar days of the date of employment. Evidence: 1. Staff #2's tuberculosis screening was dated 10/17/2019. Date of hire was 10/14/2019. Staff # 1 verified the date of employment and date of tuberculosis screenings. She was not aware of the more stringent Subsidy requirement and was following the licensing standard.
Plan of Correction:	Moving forward, will make sure staff have completed screening prior to employment.
Standard #:	22VAC40-665-580-B
Description:	Based on review of staff records, the center did not ensure staff completed Virginia Preservice Training for Child Care Staff within the first 90 days of employment or subsidy vendor approval. Evidence: 1. Staff #3, hired 08/19/2019, did not have documentation of completing Virginia preservice training within 90 days from hire. 2. Staff #1 confirmed that Staff #3 was still working on training.
Plan of Correction:	Staff #3 will finish training. Moving forward, will ensure 90 day deadline is met.
