Comments:
Thank you for your time and assistance with the unannounced monitoring inspection on December 12, 2019. This on site visit was from 9:20 a.m. through 12:30 p.m. There were nineteen (19) children in the care of four staff in three classrooms. Five children?s and six staff records were reviewed and updated. One medication and authorization paperwork was reviewed. Areas observed were: free choice activity, books, hand washing/diapering procedures, lunch, and nap. Interactions between the children and staff were observed as appropriate and the children seem comfortable with the daily routine and schedule. Violations:
Standard #: 22VAC40-185-130-A
Description: Based on a review of children's records, the provider did not ensure each record contained documentation of immunizations prior to attending the child day center.

Evidence: 1. Immunizations for Child #4 were dated 12/14/18. First date of enrollment for Child #4 was 11/6/18. 2. Immunizations for Child #5 were dated 12/14/18. First date of enrollment for Child #5 was 11/6/18. 3. The director confirmed this information, explaining she thought she had thirty days from start date to have them in the file.

Plan of Correction: Remind staff that immunizations are required prior to start date, and that physicals are due within 30 days of enrollment

Standard #: 22VAC40-185-130-B
Description: Based on review of children's records, the center did not obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence: !. The most recent documentation of immunizations for child #4, 15 months in age, was dated 12/14/18, exceeding the requirement to obtain updated immunizations every six months for children under two years. 2. The most recent documentation of immunizations for child #3 was dated 4/28/17. Child's date of birth was 3/13/17

Plan of Correction: Will request up to date immunization records for child #3 and #4. Moving forward, will request updated immunization records in a timely fashion for children under the age of two.

Standard #: 22VAC40-185-440-B
Description: Based on observation and verification from staff, one crib was not labeled for use by specific children.

Evidence: 1. There was one crib which was not labeled with the name of the child sleeping in it. 2. Staff confirmed whose crib it was and that it was not labeled.

Plan of Correction: Staff immediately labeled the crib and will make sure that all cribs have a label on the crib instead of on the wall.

Standard #: 22VAC40-185-510-E
Description: Based on observation and interview, the center failed to ensure a medication was labeled with the child's name, the name of the medication, the dosage amount, and the time or times to be given.

Evidence: 1. An Albuterol inhaler was found in a medication bag for child 1M. The inhaler was not in the original box with the prescription label and did not contain any other label. 2. The administrator was not aware of the medication and was unable to produce the original box or label.

Plan of Correction: Parent will be contacted by end of day to provide packaging that includes prescription label from pharmacy
