63.2(17) License & Registration Procedures

Comments:
An unannounced renewal inspection was conducted on 12/13/2019 from 12:20 pm - 3:30 pm. At the time of entrance there were 69 children in care with 12 staff. The children ranged in age from infant - four years. Children were observed during nap. Children in the infant room were observed playing with staff and napping in their cribs.
Records were reviewed for eight children and eight staff.
Violations were observed in four parts of the CDC standards and background checks. These violations are listed on the violation notice and were reviewed with the program director at the conclusion of the inspection.

Violations:
Standard #: 22VAC40-185-130-B
Description: Based on record review and interview, the center failed to obtain documentation of additional immunizations once every six months for children under the age of two years.

Evidence:
1. The most recent immunizations on file for child 1 were administrated on 03/20/2017.
a. Child 1 is now over the age of two years however, the facility did not obtain updated immunizations as required when child 1 was under the age of two years.
2. Administrative staff verified that an updated immunization record had not been obtained for child 1.

Plan of Correction: The parent of child 1 will be asked to provide an updated immunization record.

Standard #: 22VAC40-185-60-A
Description: Based on record review, the center failed to ensure that children's records contain all of the required elements.

Evidence:
1. The enrollment record for child 1 lacked a second emergency contact.
2. The enrollment record for child 2 lacked an address for the second emergency contact.

Plan of Correction: Complete emergency contact information will be obtained for child 1 and child 2.

Standard #: 22VAC40-185-380-A
Description: Based on observation, the center failed to ensure that there shall be a posted daily schedule that allows for flexibility as children's needs require.

Evidence:
There was no posted daily schedule in the three year old classroom.

Plan of Correction: The schedule was taken down in order to provide a copy to a contract employee.
The schedule will be posted back on the bulletin board in the classroom.

Standard #: 22VAC40-185-520-C
Description: Based on review, the center failed to ensure that when diaper ointment is used, the container shall be labeled with the child's name.

Evidence:
Three sample size containers of diaper ointment, in the two-year old classroom, were not labeled with the child's name.

Plan of Correction: The diaper ointment was labeled during the inspection.

Standard #: 63.2(17)-1720.1-A
Description: Based on record review and interview, the center failed to ensure that all applicants shall undergo a background check every five years.

Evidence:
1. The most recent child protective services central registry check for applicant 1 was conducted on 08/15/2014.
2. Applicant 1 confirmed that an updated central registry check was not available.

Plan of Correction: An updated central registry check has been requested.
