Comments:
An unannounced monitoring inspection was conducted today from 4:00pm -6:30pm. There were 33 children directly supervised by 2 staff. The physical plant, 5 children?s records, evacuation drills, emergency supplies, and policies were inspected. Children were observed participating in group play in the gym, working on crafts, and coloring. There were no medications on-site during the inspection. The cafeteria was found to be clean, safeguarded appropriately, and adequately supplied with equipment and supplies. Areas of non-compliance are identified on the Violation Notice.

Standard #: 22VAC40-185-130-A
Description: Based on record review, the center did not obtain documentation of immunizations before a child's first day of attendance.

Evidence:
Child#2's record does not contain documentation of immunization records.

Plan of Correction: I will request the documentation form the parents.

Standard #: 22VAC40-185-140-A
Description: Based on record review, the center did not obtain a physical examination by or under the direction of a physician within 30 days of attendance.

Evidence:
Child#2's record did not contain a physical examination, based on interview the child has been attending for over a month.

Plan of Correction: I will request the documentation form the parents.

Standard #: 22VAC40-185-60-A
Description: Based on record review, children's records did not contain all the required information.

Evidence:
Child#2's record is missing the parents' work phone numbers. Child#3's record is missing the father's work phone number. Child#5's record is missing the parent's work phone numbers, and the street name and the street number for both emergency contacts.

Plan of Correction: I will go to the office to complete the information, if they don't have it, I will request for the office to email the parents.

Standard #: 22VAC40-185-350-E-5
Description: Based on interview and record review, the program did not maintain staff to children ratios for children from age of eligibility to attend public school through eight years, one staff member for every 18 children.

Evidence:
The attendance sheet maintained by the after school program revealed that the center did not meet the required staff-to-children ratio. There were 37 children in care with only two staff members from 2:30 pm to 3:35 pm

Plan of Correction: I will send an email to the office.

Standard #: 22VAC40-185-540-C
Description: Based on observation, the first aid kit did not include all the required components.

Evidence:
The first aid kit was missing a second triangular bandage.

Plan of Correction: I will ask the office to give me the missing supplies.

Description: Based on review of emergency supplies, the program's battery operated radio was not in working condition.

Evidence:
The site supervisor was unable to turn on the radio during the inspection, the radio was not in working condition.

Plan of Correction: I will ask the office to give me the missing supplies.

Standard #: 22VAC40-185-550-D
Description: Based on review of emergency drills, documentation of emergency drills was not available for review during the inspection.

Evidence:
Documentation of practiced evacuation drills and shelter-in-place drills was not available during the inspection. The site supervisor stated that drills were practiced but the documentation was sent to the YMCA main office.

Plan of Correction: I will keep a copy of the emergency drill on-site.

Standard #: 22VAC40-185-560-F
Description: Based on observation, a menu listing foods to be served for snacks during the current one-week period was not posted.

Evidence:
The menu for the current one-week period was not posted.

Plan of Correction: We will print a monthly menu and post it.
