Technical Assistance:
Today we discussed the railroad ties on the playground.

Comments:
An unannounced monitoring inspection was conducted today from 3:10pm - 5:00pm. There were 42 children in care ages two months through ten years old with seven teachers, one and the owner. Children and staff records were reviewed. Required postings, annual inspections, emergency evacuation drills, first aid kit, medication and transportation were reviewed. A walking of the center inside and outside was completed. Children were observed engaged in teacher and self-directed activities including restroom routines, transitions hand washing, snack, and learning play activities. There were not any violations identified during this inspection. Violations regarding children?s records, emergency drills, medication administration certification, safe sleep, infant emergency supplies and snack were identified and appear on the violation notice. The violations were discussed with the owner at the conclusion of the inspection.

Standard #: 22VAC40-185-60-A
Description: Based on record review and interview the center failed to ensure that each child's records contains all required information.
Evidence:
1 - Four out of eight children's records reviewed do not contain parent work phone number.
a. Child 1's first date of attendance was September 10, 2018.
b. Child 2's first date of attendance was July 9, 2018.
c. Child 3's first date of attendance was May 29, 2018.
d. Child 4's first date of attendance was July 24, 2018.
2 - Staff 1 confirmed that the information was not present in the children's records.

Standard #: 22VAC40-185-240-D-4
Description: Based on medication review and interview the center failed to demonstrate that any child for whom emergency medications (such as but not limited to albuterol, glucagon, and epipen) have been prescribed shall always be in the care of a staff member who meets the requirements in subdivision 1 of this subsection. (Medication Administration Training-MAT)
Evidence:
1 - Child 5 was present for until 3:46pm. However, the only emergency medication (EMAT) trained was not present.
2 - Staff 1 stated that the only MAT trained staff went home after lunch.

Plan of Correction: A Second staff member will be MAT Trained by the end of January, 2020

Standard #: 22VAC40-185-340-A
Description: Based on observation the center failed to demonstrate the when staff are supervising children, they shall always ensure their care, protection, and guidance.
Evidence:
1 - Upon arrival there were 15, three and four year old children playing on the sidewalk next to a parking lot. Children were observed being chased by staff as they ran away from the group towards the parking lot that is used by neighboring businesses.
2 - Child 6, a three year old, was able to run away from a group of children who left the classroom to use the restroom. Unsupervised child 6 ran from the bathroom to the classroom where he ran into the classroom door, hitting his lip and causing it bleed. The staff 3, who was responsible for the child, did not see the incident. She arrived to classroom as the child's bleeding lip was discovered. The child was crying.

Plan of Correction: Staff were informed that they are no longer able to play on sidewalk and will practice proper supervision skills in the future.

Standard #: 22VAC40-185-370-1
Description: Based on observation and interview the center failed to ensure that when an infant falls asleep in a play space the child may remain in that space if comfortable and safe.
Evidence:
1 - Child 7, a seven month old infant, fell asleep during tummy time. The infant was sleep on his stomach, on top of soft filled apparatus, which was placed on top of fiber filled blanket.
a. There were not any staff with reach of the sleeping infant.

Plan of Correction: Staff will ensure that all babies are comforted and safe at all times. If a baby falls asleep on tummy time the baby will be moved to a safe position and place.

Standard #: 22VAC40-185-500-B
Description: Based on observation and interview the center failed to demonstrate that they ensure the immediate safety of a child during diapering.
Evidence:
1 - Staff 2 stepped away from the diaper changing table while a child was laying on top of the table waiting to changed.
2 - Staff 2 confirmed that she stepped away from the table after laying a child in care on top of the table to be diapered.

Plan of Correction: Staff will make sure they have all supplies before placing child on diaper changing table.

Standard #: 22VAC40-185-550-D
Description: Based on record review and interview the center failed to implement a monthly practice evacuation drill.
Evidence:
1 - Documentation provided show the last emergency evacuation drill was conducted on October 7, 2019.
2 - Staff 1 stated that she did not know if an emergency evacuation drill had been conducted in November 2019.

Plan of Correction: A fire drill was done in November; however, the director did not document the fire drill. The fire drill was recorded and the director will ensure all drills are recorded in the future.

Standard #: 22VAC40-185-560-N
Description: Based on observation and interview the center failed to demonstrate compliance regarding no child being allowed eat while walking around.
Evidence:
1 - Child 8 was observed walking around the school age classroom while eating a chocolate chip cookie.
2 - Staff present stated that the child got on the bus eating the cookie. She further stated that due to the needs of the child he was allowed to have the cookies while being transported and once he entered the classroom.

Plan of Correction: Staff will remind all children they need to stay seated while eating and will be redirect children if they try to walk around while eating.

Standard #: 22VAC40-185-570-I
Description: Based on observation and interview the center failed demonstrate compliance regarding a one day's emergency supply of disposable bottles.
Evidence:
1 - There are four hard plastic, non disposable bottles in the infant room emergency kit. However, two of the non disposable bottles appear dirty with dried substances inside the bottles.
2 - Staff present in the infant room stated ten of the enrolled infants are bottle fed. Therefore, there was not enough bottles to meet the needs of all the infants in care, in an emergency situation.

Plan of Correction: A staff member was assigned to emergency supplies and will ensure that all babies have all necessary emergency supplies by 1/10/2020.
