An unannounced monitoring inspection was initiated on 9/24/2019 in response to a self-reported incident that occurred on 9/19/2019. The details of the self-report indicated that a child was injured on 9/19/2019 when a one riser stage hit the child in the head. Follow up interviews were conducted on 10/16/2019 and 11/04/2019. Based on staff interviews and review of photographs submitted by the facility, the report resulted in a violation of not maintaining the physical plant in a safe condition. Standard #: 22VAC40-185-270-A Description: Based on staff interviews and review of documentation, the facility failed to maintain all areas and equipment inside the center in a safe condition. Evidence: on 9/19/2019, the threes classroom was playing in the classroom used as a gym. A child knocked into a "stage" that was propped up against the wall. The stage fell over and hit Child A, age 3, in the head. The stage is one riser tall and measures approximately 3 feet by 8 feet. Plan of Correction: Not available online.
