Comments:
An unannounced Monitoring Inspection was conducted today from 11:15am to 1:10pm. There were 78 (Infants to School-Age) directly supervised by 12 staff. The physical plant, 3 staff records, 10 children?s records, 3 children's medications and medication authorization records, evacuation drills, injury reports, emergency supplies, and policies were inspected. Children were observed participating in group play, watching a video, eating lunch, and preparing for and taking naps. Diapering and hand-washing procedures were also observed. There was a sufficient supply of books, toys, and materials for the children. There was an adequate number of staff with current certification in MAT, CPR and First Aid. Staff-to-child ratios were in compliance. The center was clean and organized. Areas of non-compliance are identified in this report. Standard #: 22VAC40-185-160-A
Description: Based on review, the facility did not ensure that each staff member shall submit documentation of a negative tuberculosis screening. Documentation of the screening shall be submitted no later than 21 days after employment and shall have been completed within 12 months prior to or 21 days after employing.
Evidence: Reviewed 3 staff records and found that Staff 3's initial TB screening was completed more than 12 months prior to employment.

Plan of Correction: Staff will obtain an updated TB.

Standard #: 22VAC40-185-60-A
Description: Based on review, the facility did not ensure that children's records were maintained with the required information.
Evidence: Reviewed 10 children's records and found that Child 2,3,4,5,6 and 8 did not have documentation of the parent's annual review/update of the child's emergency contact information.

Plan of Correction: Will have parents sign off to confirm updated information.

Standard #: 22VAC40-185-280-B
Description: Based on observation, the facility did not ensure that hazardous substances, such as cleaning materials, are kept in a locked place using a safe locking method that prevents access by children.
Evidence:
1) Observed in the Pre-K 1 (Tokyo) room a bottle of bleach water, soap water, febreeze and disinfectant spray in an unlocked cabinet.
2) Observed in the Preschool classroom, bleach water and soap water not in a locked place.

Plan of Correction: Will install a safety lock today.

Description: Based on observation, the facility did not ensure that electrical outlets have protective covers.
Evidence: Observed in the Toddler Classroom (Cairo) a non-safety outlet without outlet covers.

Plan of Correction: Will install outlet covers, will confirm with school maintenance to find out whether or not it is a safety outlet.

Description: Based on observation, the facility did not ensure that each restroom area provided for children is equipped with soap, toilet paper, disposable towels or hand dryers.
Evidence: Observed in the gym bathroom, that the soap dispenser was out of soap.

Plan of Correction: Staff put some in the dispenser.

Standard #: 22VAC40-185-510-J
Description: Based on observation, the facility did not ensure that medications, including staff's personal meds, are kept in a locked place using a safe locking method that prevents access by children.
Evidence: Observed in the Pre-K 1 classroom (Tokyo) Advil liquid gels in an unlocked cabinet.

Plan of Correction: Staff removed medication and put in their vehicle.
