Comments:
A Subsidy Health and Safety inspection was conducted on 12/30/19 to review supplemental health and safety requirements. An unannounced renewal inspection was completed on 12/30/19. There were eight children present with three staff present. Five children's records and four staff records were reviewed. One emergency medication was reviewed. All policies and procedures were reviewed - staff, parent, and emergency. The inspector arrived at the facility at 8:45 am and departed at 12:30 pm. The children were observed during free play time in the gym, during various small group activities, and during lunch. The children were planning to go swimming after lunch.

A licensing inspection was also conducted this date; the two inspections combined cover all subsidy health and safety requirements. Only subsidy requirements that are not covered in licensing standards are reviewed during this supplemental inspection. Violations were found during the supplemental subsidy health and safety inspection requirements and are identified on the violation notice.

Standard #: 22VAC40-665-520-B-11
Description: Based on record review and interview, the center failed to ensure that there was a written allergy care plan for each child with a diagnosed allergy to include instructions from a physician regarding the food to which the child is allergic and steps to be taken in the event of a suspected or confirmed allergic reaction.

Evidence:
1. There was documentation on the most recent physical examination for child #3 completed by the physician that child #3 had allergies to all nuts. The child had an Epipen for this documented allergy. There was no allergy care plan for child #3's diagnosed food allergy.

2. Staff #4 verified that there was no allergy care plan completed by the physician.

Plan of Correction: The child care director will contact the child's school nurse to get a copy of the school's allergy plan.
