Comments:
An unannounced monitoring inspection was completed on Monday, December 16, 2019 to determine the center?s compliance with licensing standards and as a part of the center?s six month probationary period. The inspection was also to follow up on an Intensive Plan of Correction (IPOC) to address previously cited violations related to the administration, staffing, and supervision. The inspector arrived at 9:40am and concluded the inspection at approximately 2:20pm. The center?s director and assistant director were present and assisted the inspector throughout the inspection. The census for today?s inspection consisted of 73 children in the direct care of 15 staff members. Additional staff were on site and available to the children in care. Upon arrival, the children were engaged in a variety of activities. Each classroom was inspected and formal observations were completed. The center is equipped with age and stage appropriate materials and equipment for the children?s use. All areas where the children receive care, inside and outside, were inspected. The center had the following posted: license, daily schedules, emergency numbers, evacuation maps, menu, special orders, and various parent information. Today?s lunch consisted of a chicken teriyaki bowl with diced chicken, WG brown rice, green beans, pineapple, and milk. The center?s first aid kit and non-emergency supplies were inspected and found complete. Transportation is provided and two buses were inspected today. During the inspection, eight children?s records and seven staff records were reviewed. Medication is administered and four medications were reviewed today. The most recent evacuation drill was documented on 11/12/2019. The most recent shelter-in-place drill was documented on 04/10/2019. One more shelter-in-place drill has been scheduled and must be completed by the end of the year. A lockdown drill was conducted on 05/29/2019.

Violations:
Standard #: 22VAC40-185-60-A
Description: Based on a review of eight children's records on 12/16/2019, the center did not ensure two records contained the required information.

Evidence: 1) The record for Child #6, enrolled on 07/01/2018, did not contain annual documentation of child updates and confirmation of up-to-date information in the child's record. The information was last updated when the child was enrolled on 07/2018.

2) The record for Child #8, enrolled on 08/20/2018, indicated the non-custodial parent is not authorized to pick up the child from the center. The record did not contain appropriate legal paperwork on file. 3) The record for Child #8 did not contain annual documentation of child updates and confirmation of up-to-date information in the child's record. The information was last updated when the child was enrolled on 08/2018.

Plan of Correction: The assistant director contacted the children's parents to request the missing information while the inspector was on site. In the future, families will be requested to update children's records at the beginning of the year and every six months thereafter.

Standard #: 22VAC40-185-70-A
Description: Based on a review of seven staff records on 12/16/2019, the center did not ensure two records contained the required information.

Evidence: 1) The record for Staff #2, hired on 08/05/2019, contained two references that were obtained on 08/08/2019. 2) The record for Staff #4, hired on 08/05/2019, contained two references that were obtained on 08/06/2019. Staff records shall include documentation that two or more references as to character and reputation as well as competency were checked before employment or volunteering.

Plan of Correction: References already obtained for both staff. Moving forward, we will be sure every requirement for staff files is submitted prior to when staff are hired and complete Virginia Preservice training.

Standard #: 22VAC40-185-510-A
Description: Based on a review of records and observation 12/16/2019, the center did not ensure that prescription medication was given to one child with parental authorization.

Evidence: 1) During the inspection, the licensing inspector observed a member of management administering a prescription medication to Child #9. 2) A review of Child #9's records revealed the center did not have written parental authorization to administer the medication on file.

Plan of Correction: The parents were contacted while the inspector was on site and medication authorizations requested again.

Standard #: 22VAC40-185-510-D
Description: Based on a review of four medications and interviews on 12/16/2019, the center did not ensure that two medication authorizations were available to staff during the entire time they were effective for two out of four children.

Evidence: 1) A prescription medication was observed for Child #8, but there was no written parental or physician authorization present for this medication. 2) A prescription Epipen was observed for Child #10, but there was no written parental or physician authorization present for this medication. 3) During interview, a member of management reported the authorizations for these medications could not be located while the inspector was on site.

Plan of Correction: The parents were contacted while the inspector was on site and medication authorizations requested again.

Standard #: 63.2(17)-1720.1-B-2
Description: Based on a review of seven staff records on 12/16/2019, the center did not ensure one staff obtained the results of a fingerprint based background check prior to hire.

Evidence: 1) The record for Staff #4, hired on 08/05/2019, contained the results of a fingerprint based background check that were dated 08/07/2019.

Plan of Correction: Already corrected. Moving forward, we will be sure fingerprints are completed prior to hire and before staff complete the Virginia Preservice training.
